Role Purpose:

To provide skilled, patient-centered care and education for individuals living with chronic and rare conditions, ensuring consistent support through virtual consultations.

Key Responsibilities:

Conduct a minimum of 3 patient consultations per week via telehealth. Provide condition-specific education and care planning with a focus on access to healthcare, for patients and families.

Collaborate with physicians and allied health professionals to coordinate care.

Document patient encounters accurately and maintain confidentiality. Offer emotional support and guidance to patients and caregivers.

Qualifications: Registered nurse (AHPRA). Experience or strong interest in chronic/rare conditions Excellent understanding of the health and disabilities sectors (including NDIS access) Comfortable using virtual health platforms Confident, empathetic, patient-focused, and adaptable

Commitment: Minimum of 3 patient sessions per week over 12 months minimum following training Ongoing participation in training and peer support meetings.
