Family Advocate

Advocacy & Support
- Provide individual advocacy for families experiencing barriers to accessing services or exercising their rights.
- Assist families to communicate with government agencies, service providers, and community organisations.
- Help families prepare for meetings, reviews, or appeals (e.g., NDIS, child protection, housing).
- Promote the rights, safety, and wellbeing of children, young people, and vulnerable family members.
Information & Navigation
- Offer clear, accurate information about relevant systems (e.g., education, disability, health, social services).
- Support families to understand policies, procedures, assessments, and decision-making processes.
- Connect families to community programs, financial support, counselling, and other services.
Case Management & Documentation
- Maintain up-to-date case notes, records, and advocacy plans.
- Manage a caseload with professionalism, confidentiality, and cultural competence.
- Monitor progress, follow up actions, and evaluate outcomes with families.
Collaboration & Community Engagement
- Build and maintain positive relationships with service providers and government departments.
- Participate in interagency meetings and case conferences.
- Contribute to community education, workshops, and outreach activities that support family empowerment.
Systemic Advocacy & Continuous Improvement
- Identify trends, barriers, and systemic issues affecting families.
- Provide feedback to management on service gaps and policy challenges.
- Contribute to program development, evaluation, and quality improvement activities.
